Finding Care Gaps that Compromise Quality and Increase Cost

SAGE is a powerful tool which allows you to identify care gaps in treatment and compare those gaps with evidence based medicine all within the context of an episode of care using ETGs. SAGE is customizable which means that the guidelines can be changed to meet your physician community’s approval. In fact, you can even take guidelines you have already developed and put them into SAGE or create new ones too! Unlike some EBM products, we designed SAGE to fit your needs, not for you to fit ours.

Care gaps occur in every specialty with virtually every disease. Quality measures from organizations like the National Committee for Quality Assurance and the National Quality Forum cover only a small percentage of the most common care gaps. The ProfSoft Standards and Guidelines Engine comes with a starter kit of over 500 built-in measures covering over 90 different medical conditions. Rather than a few quality measures focused on prevention, SAGE provides a wide array of measures covering all aspects of health care from prevention to routine primary care to complex specialty care.

A Medicare managed care organization used SAGE to profile for bacterial pneumonia. Their clinical standard called for antibiotic treatment within the first five days.

The bar chart (left) shows that only 14% of patients had to be admitted when antibiotics were prescribed, compared to 31% when antibiotics were not prescribed. Total episode costs were higher also.

For these ‘care gaps’ SAGE identifies exactly which patients and physicians need education.
